Amended Local Safeguarding Children Board Northamptonshire Procedures for Serious Case Reviews
Please be advised that Northamptonshire Serious Case Review Procedures are currently being rewritten to reflect the publication of Chapter 8, Working Together to Safeguard Children March 2010 (DCSF). In the meantime partner agencies are advised to use this latest statutory guidance until the Northamptonshire Inter-agency Child Protection Procedures have been rewritten.
The Review Process
Serious Case Reviews are conducted in accordance with Chapter 8 Working Together To Safeguard Children and Northamptonshire Inter Agency Child Protection Procedures.
When a child dies and abuse or neglect is known or suspected to be a factor the Local Safeguarding Children Board is required to carry out a serious case review.
The purpose of serious case reviews are to:
- Establish whether there are lessons to be learned from a case about the way in which local professionals and agencies work together to safeguard children;
- Identify clearly what those lessons are, how they will be acted upon and what is expected to change as a result, hence improve inter-agency working and better safeguard children.
Case reviews are not enquiries into how a child died or who is culpable; that is a matter for Coroners and Criminal Courts respectively to determine, as appropriate.
Each Serious Case Review has a membership of very experienced senior managers from the LSCBN partner agencies. Overview panel members are identified according to the needs of each case to ensure that the panel is independent of involvement in the case and has access to any expert knowledge required. Recommendations are made by the Overview Author and presented to the LSCBN, who take responsibility for developing an action plan.

Serious Case Review Bulletin
The LSCBN Serious Case Review Sub Committee have published a bulletin reflecting on serious case reviews that Northamptonshire have either commissioned or contributed to. Individual cases are not identified or commented upon. The aim of the bulletin is to inform practitioners and improve practice in Northamptonshire. The latest editions can be found below.

Executive Summaries
In working transparently the LSCBN also commissions the Overview Author to prepare an executive summary of each Serious Case Review. Names of all subjects are made anonymous to protect their identity.
Executive summaries of serious case reviews are available for you to view/download.
